Home and Community Based Services (HCBS)

Attendant Care

We offer a member assistance in their home, work, or community activities. The assistance provided is individualized based on the needs of the member.

Habilitation

Habilitation services assist members in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings.

Respite

We provide short-term care and supervision consistent with the health needs of the member to relieve the primary caregivers, so they may have time and space to accomplish needed tasks that might otherwise be difficult.

A center-based employment program where we focus on the skills necessary for members to gain meaningful employment.

Group and Individual Supported Employment

Employment services that provide members with an on-site supervised work environment in a group or individual setting. Members are paid by Lexington Learning Center or community employers for work performed in accordance with State and Federal Law.

Enrolling is Easy

We will do our best to find the right provider for your family. Please let us know if you have any preference for your provider, such as male or female, etc. We want to work with your desired time frame, whether it be Monday – Friday, only on the weekends, or perhaps just after work. Knowing your family’s location will help us find a provider in that area or transportation if needed.
