Lexington Behavioral Health Services (LBHS) provides professional and tailored care for both children and adults with behavioral and developmental disabilities.

Our residential services provide an alternative to hospitalization and out-of-state placement. Our unique transitional program offers Step-Up/Step-Down services that adapt to members’ individual needs.

By providing a continuum of supports that work together we can effectively provide timely short and long-term placement options, individualized intense wrap-around support services, and transitional planning that leads to member choice and inclusion in a least restrictive environment.

What makes LBHS special?

One of the things that makes LBHS unique is the way we have blended Psychosocial Rehabilitation with an ABA framework to create a more comprehensive and holistic member support plan.

The goal of psychosocial rehabilitation is to teach emotional, cognitive, and social skills that help those diagnosed with mental illnesses live and participate in their communities as independently as possible. Ensuring members are engaging with their environment and gaining natural supports as well as advocating for family support are two components of psychosocial rehabilitation that LBHS emphasizes.

LBHS integrates an ABA framework by creating specific treatment plans that include mastery criteria and measurable broken down steps. This allows progress to be measured and helps us see the effectiveness of interventions.

What is the LBHS Patient Population?

We work with developmentally impaired / delayed populations that also have co-occurring severe mental illness / complex behavioral support needs.

What is our specialty?

          We specialize in working with patients who are dually diagnosed = developmentally impaired and complex behavioral health diagnoses.

What is the LBHS patient population age range?

We primarily work with adolescents and young adults (5yrs – 35yrs)

What environment does LBHS typically serve patients in?

At home or out in the community and often both. We work inside group home environments as well as natural home environments. We also go out into the community with our members on an as needed basis.

What other agencies do we work with?

LBHS works alongside DDD providers such as community habilitation services and habilitation residential services. LBHS also works alongside DDD day programs and other center-based DDD programs and services.

What Health insurance providers does LBHS take?

LBHS only accepts AHCCCS Managed Care Organization insurance. Currently we are enrolling for both AHCCCS United Health Care Community plan and Mercy Care of AZ DDD plan.

Does LBHS accept private pay or private insurance?
No LBHS is specifically a Medicaid / AHCCCS program.
Where in Arizona does LBHS provide services?
Only in the greater phoenix metropolitical area and the surrounding east valley communities.
How can I access LBHS services?
All LBHS referrals start with the patient’s health insurance representative. If your patient currently has UHC DDD community plan by AHCCCS and has complex behavioral needs combined with a developmental disability contact your insurance health insurance rep and ask for Case Manager to be assigned. If you have a High Needs Care Case Manager already assigned, speak with them about options for intensive outpatient behaviorally complex supports such as a Meet Me Where I Am program and ask for Lexington Behavioral Health Services. Only a High Needs Case Manager from an AHCCCS approved organization can submit a referral for LBHS outpatient services.
